East Arkansas Family Health Center (EAFHC) provides comprehensive primary and preventive health care services appropriate for the population served at each of our health centers. We offer medical, dental, pharmaceutical, ophthalmology, laboratory, radiology, and transportation services. We also provide preventive health screenings, diabetes and cardiovascular wellness and nutrition classes, HIV/AIDS care, and behavioral health assessments. We are located in Crittenden, Poinsett, Mississippi, and Phillips counties, with a total of seven primary care medical offices and an HIV/AIDS care center.
This health center receives Health and Human Services (HHS) funding and has Federal PHS deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
Accredited by The Joint Commission, EAFHC has developed a reputation of excellence and is considered one of the premier organizations providing a patient-centered medical home to people in eastern Arkansas who would otherwise be without access to quality primary health care services. EAFHC has also been recognized as a "Health Center Quality Leader" by the Health Resources and Service Administration (HRSA).

What is it and how does it affect me, as a patient?
Patient-Centered Medical Home (PCMH) is an approach to providing total health care and putting you, the patient, at the center of your health care. Under the PCMH model of care, you will join a team that includes your provider of choice, a care team, trusted family and friends (if you wish), and most importantly, you. With this new relationship, you will be informed and involved in your health care decisions that bring together you, your family, and your health care team to make the best choices about your health.
East Arkansas Family Health Center is recognized by the National Committee for Quality Assurance (NCQA) as a Patient-Centered Medical Home at five of our primary care clinics (in West Memphis, Blytheville, Helena, Lepanto, and Trumann). Family Health Center In Arkansas
We treat patients of all ages who are insured, uninsured, and traditionally underserved. Although we are not a free health center, we do offer services at a discounted price for those who qualify based on their income and household size. Services will not be denied due to the inability to pay. Learn more about our sliding fee scale and eligibility. We also provide transportation to and from each location for patients who need additional services not performed at the clinic, such as mammograms.
Not only do we diagnose and treat acute illnesses and injuries, such as the flu and minor fractures, but we also treat and manage chronic diseases such as diabetes, hypertension, and obesity. Additionally, we provide OB/GYN services for girls and women, as well as pediatric care for newborns to adolescents.
In addition to primary care and dental care, EAFHC also offers diagnostic services such as blood draws and X-rays at each of our locations, as well as a full-service pharmacy at our West Memphis and Lepanto offices. One day a week, at our West Memphis office, EAFHC has an on-site cardiologist from Southern Cardiology. In addition, we offer ophthalmology services at our West Memphis clinic.
